Pontine Syphilitic Gumma in an HIV-negative Patient

The patient was a 24-year-old man who presented with a 2-month history of headache, gait disturbance and hearing loss. He had no skin or genital manifestations before these symptoms. T2-weighted magnetic resonance imaging (MRI), fluid-attenuated inversion recovery MRI, contrast-enhanced T1-weighted MRI, and diffusion-weighted MRI revealed a mass with extensive edema and dural enhancement in the pons (Picture, respectively). Although cerebral syphilitic gummas are generally considered to be rare manifestations of tertiary syphilis, many reports exist of early cerebral syphilitic gumma.
